About the role:

Clinical Care Guides support our Case Managers through day-to-day tasks that positively impact the member experience and clinical intervention. Through assistance with care coordination and care routing the team helps members reach their optimum health goals in the most cost effective way . Assisting members throughout all phases of their clinical journey through answering questions and resolving issues and navigating the complexity of the healthcare system.

You will report into the This is a remote / work-from-home role. You must reside in one of the following states: Arizona, California, Connecticut, Florida, Georgia, Illinois, Maryland, Massachusetts, Michigan, New Jersey, New Mexico, New York, North Carolina, Ohio, Oregon, Pennsylvania, Rhode Island, Tennessee, Texas, Utah, Virginia, or Washington. Note, this list of states is subject to change.

Responsibilities:

• Triage Clinical Alerts and create Inpatient tickets
• Triage Clinical Intervention tickets and assist with providing member resources
• Communicate with hospital staff to get member information
• Coordinate with Concierge Case Managers to assist with discharge planning
• Assist in the coordination of care across a variety of settings (hospitals, outpatient clinics, home care, supply vendors, dialysis) while maintaining strict confidentiality and the highest level of professionalism
• Send HIPAA authorization requests and other documents to facilities/hospitals to assist in guiding members INN

Requirements:

• 3 years of Customer Service Experience or Bachelor’s Degree
• Intermediate computer literacy skills
• Bilingual (English/Spanish) Written and Verbal

• Prior healthcare or Service experience
